I have read the check list page, the rules and regs
page and all other policies mentioned on this site
prior to mailing in my application, and I understand
this is part of my contractual agreement.
The 8 foot booths are 8 deep, with a 12 foot frontage (single
booth) 18 foot frontage for a booth and half, and so forth.

Booth Size and fees are as follows:
A. Single booth 10 x 10 or 8d x 12w $305.00
B. Booth and Half 10 x 15 or 8d x 18w $452.50
C. Double Booth 10 x 20 or 8d x 24w $610.00
D. Half Booth 8 deep x 6 wide (new) $185.00
NO set-up fee as in the past.
A Booth in Commons Area: Please add $25.00 for a single booth, $38.00 for a booth & half, or $50.00 for a double. This
MUST BE a separate check.

(If Jewelry sales is unmarked or marked "no" you will NOT be allowed to sell jewelry. If marked "Yes" you still must be approved.
There is only a limited amount of electricity, this is first come first serve when applying to show. 40 Watt Bulbs only/200 watt Max per booth.
Deadline: July 15 or when category closed. Cancellations: If you cancel prior to July 15 you will receive booth fee less $75.00 cancel fee. If
cancel after August 10, you will receive refund of booth fee less $100.00 and if you cancel after September 5, no refund will be given,
however please be sure to call CCS and let them know that you will be unable to attend. If you cancel and do not notify us, you will be taken
off of our mailer. Please be considerate of the other vendors in the show and call if your a no show. There is no sub-leasing of booths. I also
understand I must be in my booth no later than 8:00am on show day, otherwise I forfeit my spot with out refund and/or if I bring any
commercial items, CCS has the right to pull items from my booth, or remove me and/or my product from show completely with out refund.
Bounced checks are a $50.00 fee. I have also read and understood the Cancellation/Refund Page, Rules and Regs page, Check list page,
and all other policies listed on website..

HOLD HARMLESS AGREEMENT: I hereby release Creative Crafters' Showcase, Inc., Noreen Guglielmo and/or Anthony Guglielmo, its
officers, board of directors and members of District 38 or LP from any claim or right for damages, which may occur to me, my employees, my
children or helpers and or products. I also assume and accept full responsibility for any damages done by us or our workers a the Creative
Crafters' Showcase, Inc. show held on the date above at Lewis Palmer High School. I assume all responsibility for the purchased booth
footage and the product we sell, holding Creative Crafters' Showcase, Inc free of all liability. I fully understand this Hold Harmless Agreement.
I carry my own insurance and liability which covers the above mentioned peoples and products that we sell.. I understand and agree to all
the terms in this contract and have read the rules and regulations, cancellation, bad check and other policies listed on the website.
I also understand , CCS reserves the right to use my photos and/or business name for ANY advertising.
